For those with insurance:

Fill out the immediately below linked form, which has questions to collect your insurance data . Your portion of the payment will be calculated and due at the time of treatment. Due to the nature of dental insurance, sometimes this is only an approximation billing. Bring your employee benefits booklet to resolve any questions about billing at your appointment. We can bill all PPO dental insurances, but not DHMOs. Dental Health Management Organization insurances (DHMO) don't allow for 'external' dentists to bill them. This is opposed to Preferred Provider Organizations (PPO) which do, most patients have PPO insurance.
