TAM Patient or Case Worker Guide
We are a U of U Dental Affiliate Provider
*Please Note: We are monitoring the new Dental rollout and will post updates here.
As of the first day (April 1), it appears that all TAM dental benefits have been converted to the "Dental Program for Adults" which is the same as everyone else receiving medicaid benefits. This means you don't have to "activate" TAM Dental benefits anymore, but patients do still have to be treated by a provider in the "U of U School of Dentistry Network." We will leave this TAM information up for a few weeks until the dust settles. In the meantime, please use the scheduling link in step 3 to schedule appointments on behalf of your clients.

*If the patient is unable to fill out the New Patient Forms we can assist in filling out new patient forms with them during their first visit. Please advise patient to bring a medication list.

Waiting For the State
Typically the State activates the available TAM benefits within days, but in some cases it may take weeks, such as over the holidays. This is out of our control, so your patience is appreciated. Unfortunately, once the available benefits are active the state doesn't immediately notify anyone. If you would like to put a patient on our "radar", call us, we can continuously check the state database to allow us to schedule. Once the patient's benefits go from available to active you can schedule the patient using the link below:

What if my patient has a dental emergency that can't wait?
As mentioned, usually it is only a few days to activate the available TAM benefits, but if there is an emergency (pain/infection), we can see a patient under their active medicaid health plan. This can allow us to issue antibiotics, take basic imaging, or extract teeth only. If this is the case please call the clinic to allow us to help with the dental emergency outside of our regular scheduling.
